Injury Checklist
1.
Provide medical treatment to employee. Does your clinic drug test automatically?
2. Perform investigation & document findings, i.e., statements, photographs, witnesses statements.
3. Complete the necessary State of Michigan and insurance forms.
4. Keep in touch with the injured worker and the medical facility.
5. Consider return-to-work with restrictions, if possible.
6. Inform claims examiner of all information you have developed.
7. If matter is litigated, contact the defense attorney and ask to be kept informed of the progress of case.
